What Is Vagifem Used For?

Vagifem vaginal tablets are from the drug class known as Hormone Replacement Therapy (HRT). It is normally used to alleviate menopausal symptoms part of the genitourinary syndrome of menopause (GSM). 

GSM tends to cause issues such as vaginal dryness, irritation inside the vagina and also around the external genital region also known as ‘vaginal atrophy’. It may also cause extreme burning and stinging. Women with GSM symptoms tend to also report higher incidences of urinary tract infections (UTI) which can also become chronic and cause a lot of discomfort and unease on a daily basis.

Who Can Use Vagifem?

Vagifem can be used by women experiencing menopausal symptoms such as the previously described genitourinary syndrome of menopause (GSM). GSM causes mild to severe forms of vaginal atrophy which can be painful and cause discomfort on a daily basis and hence Vagifem can be used to provide relief from these symptoms by providing moisture and vaginal lubrication alongside other benefits.

How to Use Vagifem Tablets?

Vagifem 10 micrograms vaginal tablets contain estradiol hemihydrate which is equivalent to estradiol 10 micrograms. It also contains the following ingredients: hypromellose, lactose monohydrate, maize starch and magnesium stearate. The film coating on the tablet is made of hypromellose and macrogol 6000. 

These are white, film coated tablets with a diameter of 6mm which are biconvex in shape and have a marking of NOVO 278 on one side. Each tablet comes with an applicator which is a single use applicator and should be discarded after use. 

Vagifem comes in 2 pack sizes:

  • 18 vaginal tablets with the applicators
  • 24 vaginal tablets with the applicators 

At the start of your treatment, your doctor will prescribe you the lowest dose of Vagifem to ensure treatment time is kept short. However, if the medication is not working for you because it is not strong enough or conversely the medication is too strong, then let your doctor know to reevaluate the treatment plan and start you on the correct dose to ensure treatment is effective.

How many tablets should I use for the treatment?

  • For the first 2 weeks: use one vaginal tablet each day 
  • Reduce to one vaginal tablet twice a week thereafter 
  • Ensure to leave 3-4 days between the doses after reducing to twice a week.

What if I use more vagifem than I should have?

  • Ensure to let your doctor or pharmacist know as soon as possible if you take more vagifem than prescribed or instructed 
  • Vagifem tablets are designed for intravaginal use hence the dose is very low. In order to achieve a similar dose effect by oral tablets, a lot more vaginal tablets would need to be used therefore using more Vagifem than needed is unlikely to harm you. 

What if I forget to take my medication?

  • If you forget to take a dose, try to take your dose as soon as you remember it.
  • However, if it is nearly time to take your next dose, skip the missed dose and take your next dose as normal.

What if I stop using Vagifem?

  • Ensure to inform your doctor before stopping Vagifem therefore continue treatment until you inform your doctor. This does not apply if you are experiencing side effects and therefore stopped the treatment. 
  • Speak to your doctor for an alternative treatment 

What If I have an upcoming surgery soon?

  • If you have a surgery coming up let your doctor or surgeon know that you are using Vagifem. 
  • Vagifem treatment may need to be paused at least 4-6 weeks before the surgical procedure.

Only use this medication under the supervision of a qualified health care professional. Treatment with Vagifem can be started as advised by your doctor or on whatever day suits you best. 

The following instructions outline how to use Vagifem tablets:

  1. When you open the pack, rip off one single blister pack on one end and pull it downwards.
  2. Position yourself as you feel comfortable (similar to inserting a tampon) and carefully insert the applicator into the vagina with the push-button facing away from you.
  3. As soon as you feel some resistance stop the insertion.
  4. In order to release the tablet from the applicator, gently press on the push button until you hear a click sound.
  5. This will release the tablet which will stick to the walls of the vagina and will not drop out when you are up and moving.
  6. Take out the applicator and discard it safely. 
  7. Do not reuse the applicator.

  • Evidence shows that Vagifem cream does not increase the risk of developing breast cancer.
  • However, it is unknown whether it is safe to use in women who have had breast cancer in the past 

Ovarian Cancer 

  • Oestrogen-only HRTs have been shown to increase risk of developing ovarian cancer which is rare compared to breast cancer. 
  • The risk of ovarian cancer also tends to increase as you age.
  • Women on HRTs for 5 years or more show 3 cases of ovarian cancer per 2000 women who use HRTs compared to 2 in 2000 women who do not use HRTs.

Endometrial hyperplasia (excessive thickening of the womb lining)/endometrial cancer 

  • This includes thickening of the endometrial lining and also cancer of the endometrial lining 
  • Taking oestrogen only HRTs over a long period of time can increase risk of developing endometrial cancer
  • However, it is unclear as to whether Vagifem can cause endometrial cancer as treatment is not normally long-term.
  • Bleeding or spotting at random times is normal however, it is recommended to check with your doctor for further assessment.

Blood clots in veins (thrombosis)

  • The risk of developing blood clots in veins is 1.3 to 3 times higher in patients who take HRTs compared to those who do not. 
  • The risk of blood clots also increases with age.
  • In the case of signs of blood clots, stop the medication immediately and refer to a doctor.

Heart disease

  • No risk of heart disease shown in women who take estrogen only HRT therapy 

Stroke 

  • The risk of stroke is around 1.5 times higher in women who use HRTs than women who do not.
  • Furthermore , cases of stroke due to HRTs are also shown to increase with age.

Other conditions 

  • Evidence of women who use HRTs and are over the age of 65 may be at a higher risk for memory loss problems.
  • It is advised to speak to a doctor for more information.

NHS prescriptions are currently free of charge in Scotland, Wales and Northern Ireland. In the UK, Vagifem is currently prescribed as a prescription only medication (POM).
